Utilización de la vía subcutánea en el ámbito domiciliario en personas que se encuentra en el final de la vida: revisión narrativa estructurada

Resumen
Introducción: Los pacientes en situación terminal o de últimos días que permanecen en su domicilio plantean algunos retos a sus cuidadores como es la administración de medicación cuando se pierde la vía oral, o cuando la posibilidad de poder coger una vía periférica se hace insostenible, todos estas incógnitas las resuelve una vía de administración alternativa, que es poco conocida en algunos casos por el personal sanitario a pesar de las múltiples ventajas que nos aporta, hablo de la vía subcutánea. Objetivo: Estudiar los beneficios del uso de la vía subcutánea en la atención domiciliaria de pacientes adultos que se encuentre en situación terminal. Metodología: Revisión narrativa estructurada de la literatura. Búsqueda en las siguientes bases de datos nacionales e internacionales: MEDLINE, CINAHL, COCHRANE, CURRENT CONTENTS CONNCECT, LILACS, CUIDEN, SCIELO y CSIC. Resultados: La mayoría de los estudios coinciden en las ventajas y beneficios que aporta el uso de esta vía de administración en los últimos días de vida, siempre que sea factible su implementación, a pesar de la falta de estudios en materia de medicación para su administración subcutánea y la escasa formación de los profesionales de la salud en este aspecto. Conclusiones: Una mejor formación por parte del personal sanitario, además de optimizar los cuidados paliativos, facilitaría e incrementaría el uso de la vía subcutánea que aporta beneficios tanto al paciente como al cuidador.
Abstract
Introduction: Patients that are in terminal phase and have decided stay at home pose some challenges to their caregivers, such as is the administration of medication when the oral way is lost or when the possibility of a peripheral venous catheter is unsustainable. In these complex situations, the subcutaneous way can be a good alternative, although it is barely known, even by the health staff despite its numerous advantages. Objective: To study the benefits of the use of the subcutaneous way in the home care of adults patients in terminal phase. Methodology: Structured narrative review of the literature. Research in the following national and international data bases: MEDLINE, CINAHL, COCHRANE, CURRENT CONTENTS CONNCECT, LILACS, CUIDEN, SCIELO and CSIC. Results: Most of the studies agree with the advantages and benefits that the use of this way of administration provides in the last days of the patient’s life, as long as its implementation is possible, even though the lack of research in terms of medication for its subcutaneous administration and the limited training of the health staff in this aspect. Conclusions: A better training of the health staff, as well as the optimization of the palliative cares, will enable and increase the use of the subcutaneous way, which provides benefits for both the patient and the caregivers.
